[ Abstract ]

  In recent years, there has been a gradual focus on prevention in medical care, including projects such as the Health Japan 21 and Health Frontier strategy. The medical-care system reform in 2006 implemented the insurers’ obligation of designated health examination and medical guidance for subscribers of medical insurance over 40 years of age. This study focuses on the roles of insurers in prevention and examines the background and challenges to such roles. The emphasis on prevention implies that the aim of medical security itself has changed to one that secures health. The characteristics of the prevention of lifestyle-related diseases are that the occurrence of the disease is greatly affected by one’s lifestyle and long term prevention measures are necessary. The prevention measures conducted by insurers have some advantages; they differ among regions and occupation and the cost consciousness of insurers is high. However, one of the future challenges is to what degree the prevention measures for lifestyle-related diseases conducted by insurers can be introduced to individual’s lifestyle, therefore it may be necessary to consider the addition of incentives through means such as insurance premium. Concerning the system design for insurers, we must seek for a medical insurance system that individual’s can continue to hold for a long period of time. Moreover, the diversity of benefits of prevention should be recognized and taken into serious consideration and the objective of prevention should not be limited to only the decrease in medical costs.
